Fees are shown at the ENTRY TIER, the band a new account trades in before any volume discount, because that is the number the reader will actually pay. Some venues print a maker/taker split and others a single headline trading fee; each is shown as printed and no blended effective rate is computed, because a derived number rendered as though a venue published it is the thing this index exists to replace. A venue whose page could not be read is listed WITH that fact and the reason, never with an estimate and never with a blank.
